Which data we process depends on your contact with our practice and on the care we provide to you.

5. Your medical record

When you are treated by us, we record the data necessary for your treatment in a medical record.
A physiotherapist, like other healthcare providers, keeps a medical record. This may include, among other things, your diagnosis, treatment, progress and relevant information that you provide yourself.
We use Fysiomanager for this purpose, an electronic patient record for physiotherapy.
Your medical record is handled carefully and confidentially. Only persons who are authorised to do so and who need access for their work can gain access to patient data.

6. Fysiomanager and MijnZorgtoegang

For our patient administration, record-keeping and various processes surrounding your treatment, we use Fysiomanager.
Through MyCareAccess, patients can, among other things, make appointments online and, when it’s part of the treatment, receive and fill out questionnaires.
MijnZorgtoegang is integrated with Fysiomanager. Completed questionnaires can therefore become directly available in the patient record.
Within our Fysiomanager package, we also use the available functionalities for, among other things, patient communication, policy verification and administrative processing.
Fysiomanager is provided by World of Health B.V. For the processing of personal data within the service, appropriate agreements on data processing and security apply. Fysiomanager states that it maintains data processing agreements for this purpose.
Fysiotherapie Braakhuis, as the healthcare provider, remains responsible for the processing of personal data within our own care provision.

7. Online appointments

Via the website you can make use of the option to make an appointment online.
When you click the button for online appointments, you are directed to the online environment of MijnZorgtoegang/Fysiomanager.
The personal data you provide there is processed within the context of the appointment and the care provision of Fysiotherapie Braakhuis.
The privacy and security measures of the relevant service also apply to the processing within this environment.

8. Questionnaires

When this is part of your treatment, we may send you questionnaires via MijnZorgtoegang.
The answers may contain health data. When this information is relevant to your treatment, the completed questionnaires may become part of your medical record.
Fysiomanager states that completed questionnaires from MijnZorgtoegang can be made directly available in the patient record.

9. Invoicing and health insurance

We process personal data that is necessary for the administrative handling of your treatment.
This may relate to, among other things:
Invoicing and administrative processing take place, among other things, via Fysiomanager.
When your treatment is declared to your health insurer, the data necessary for this may be provided to the relevant health insurer or other party when this is necessary under laws and regulations.
We do not provide more data than is necessary for the relevant purpose.

You can contact us via our website.
When you use the contact form, the data you fill in is sent to our business email address info@fysiotherapiebraakhuis.nl.
We use this data solely to answer your question or to contact you.
Please preferably do not use the general contact form and regular email to send extensive medical or other sensitive health information. For medical information we use the communication channels designated for this purpose within our practice.

11. Google Workspace

For business email we use Google Workspace, in which we use the email address info@fysiotherapiebraakhuis.nl.
Personal data that you share with us via email may therefore be processed within this business email environment.
We only use email insofar as this is appropriate for the relevant purpose. Additional secure communication channels may be used for exchanging medical information.

12. Disclosure to other healthcare providers

We do not simply share your medical data with other persons or organisations.
When it is necessary for your treatment to share relevant information with another healthcare provider, this is done in accordance with applicable legal rules. When your consent is required, we will ask for your consent.
There may be situations in which data may or must be provided without your consent because a legal obligation or legal exception exists for this.
We only provide data that is necessary for the relevant purpose.

13. Security

We take appropriate technical and organisational measures to protect your personal data and medical data against loss, unauthorised access, unauthorised use, alteration or disclosure.
Access to patient data is limited to persons who need this data for their work.
Our digital care environment uses systems that are specifically designed for the processing of care data. Fysiomanager has published information security certification according to NEN 7510 for the development, hosting and support of Fysiomanager and MijnZorgtoegang.

14. Retention period medical record

We retain your medical record in accordance with the statutory retention periods.
As a general rule, medical records are retained for at least 20 years from the last change in the record. Under certain circumstances, a record may be retained for longer.
Other personal data may have a different statutory retention period.

15. Your privacy rights

You have various rights with regard to your personal data and medical record.
You have the right to, among other things:
Your right to erasure under the GDPR does not mean that medical data must always be deleted immediately. Statutory retention periods and specific rules apply to medical records.
